The Standards of Practice for Case Management were first introduced by the CMSA in 1995 and then revised in 2002. We are pleased to offer the Standards of Practice for Case Management, 2010 revision, which provides voluntary practice guidelines for the case management industry. The Bradley Corporation business model is to go to the next level in business services. We believe that these are all important factors that case managers need to address in their practices. The Standards of Practice are intended to identify and address important foundational knowledge and skills of the case manager within a spectrum of case management practice settings and specialties. The Bradley Corporation Standards reflect many changes in the industry, which resonate with current practice today. Some of these changes include the following:

  • Minimizing fragmentation in the health care system
  • Using evidence-based guidelines in practice
  • Navigating transitions of care
  • Incorporating adherence guidelines and other standardized practice tools
  • Expanding the interdisciplinary team in planning care for individuals
  • And improving patient safety

Principal Term for Case Management: The context in which case managers provide health and human services to clients and their support systems. The process consists of several steps or sub-processes that are iterative, cyclical and recursive rather than linear in nature and applied until the client's needs and interests are met. The steps include screening, assessing, stratifying risk, planning, implementing, following-up, transitioning, post-transitioning communication, and evaluating outcomes. The process, with special intervention by case managers, work together with clients and their support systems to evaluate and understand the care options available to the clients; identify what is best to meet their needs; and institute action to achieve their goals and meet their interests and expectations.

The Bradley Corporation Case managers are recognized experts and vital participants in the care coordination team who empower people to understand and access quality, efficient health care. We work to collaborative process of assessment, planning, facilitation and advocacy for options and services to meet an individual's health needs through communication and available resources to promote quality cost-effective outcomes.

Employee Assistance Support

EAS

We are dedicated to serve improves the lives of the unsupported and veterans by offering services to those in need. The Bradley Corporation works to develop and facilitate community actions that move homeless individuals and families to self-sufficiency and permanent housing. Through the generosity of many grant funders and sponsors, we are able to provide emergency shelter and family support programs at no cost to the community. However expenses such as clothing, food, utilities and maintenance of the shelter are not always covered. Therefore we greatly depend on community support to help maintain all of the shelters and soon to be a children nursery. All of this would not be possible without your support. The Military Program assists American Military families of all branches and veteran status who have unmet needs due to death, injury, physical or mental detriment, financial hardship or in any crisis situation that arises as a result of their service. We hope to do this by identifying the service needs of the homeless community and to work with organizations to coordinate, create and develop resources, which would facilitate these community actions into a reality of actual services offered.

CHS

The Bradley Reid Corporation, a charitable, tax-exempt, 501(c)(3) organization, is dedicated to promoting empowerment of people, and the foundation focuses its funding on committed, grass roots organizations that rely strongly on volunteer efforts and where foundation support will make a significant difference. Our mission is to restore pride in our veterans by helping them get off the streets and into the workforce. The Bradley Reid Corporation (BRC) is a multi-service community organization that fulfills its mission of helping many, one by one through dynamic programming responsive to the needs of diverse communities. BRC's services include peer education, training and outreach; specialized women's services; health promotion and harm reduction initiatives; links to medical care and appropriate housing; translation assistance and child care support; HIV treatment education; supportive counseling; and much more. These comprehensive programs promote the well-being, empowerment, and stability of persons living with and at risk for HIV/AIDS throughout the City and the neighboring areas. The mission of the BRC of Metropolitan Charlotte is to develop the total person-spirit, mind and body-through character-development programs that build strong kids, strong families, and strong communities.


Community Crisis Intervention Services

CIS

As an organization, we are committed to do whatever it takes to help our veterans and our community. Teaming up with partnerships and this program gives us another way to bridge the gap.” The Bradley Reid Corporation Veterans Support Services raises funds that go directly to the support of veterans in need. No professional fundraisers are hired by the organization. Veterans and non-veterans are part of an on the job fundraising training program. The Bradley Reid Corporation provides a source of additional support by partnering with other organizations throughout the U.S. whose mission is to help our soldiers in any way.

The Bradley Reid Corporation is a 501C3 not-for-profit that helps veterans, who are homeless or near homeless, organize a “path” to independent living. The organization has created and manages several programs that specifically address the emotional and financial needs of veterans.

Peer Support Services

PSS

"Guiding Kids to a Bright Future"

Counseling & Prevention programs provide individual, group and family counseling to address issues such as: adjusting to a new school environment, improving behavior and interpersonal relations, coping with grief or loss, handling environmental stress and regulating emotions. Our staff works to identify students in need of counseling services, track the delivery of those services, and provide periodic feedback on student progress. We offer mentoring, violence prevention and educational enrichment program that promotes social, emotional and behavior competencies in at risk male youth. We address six core values that relate to both personal and academic success: integrity, accountability, self-determination, positive anger expression, visionary goal-setting and respect for women.

We will help youth to be more effective in meeting challenges in life, and preparing them for the future. We believe every youth has the potential to become a good leader, but this potential sometimes needs to be developed. Our aim is to cultivate youth in the community to become strong leaders, to choose positive lifestyles and to make wise decisions:

  • by encouraging high scholastic and ethical standars;
  • by improving social status through community involvement;
  • by building character and promoting unity through the development of relationships amond peers;
  • by developing responsible individual through financial literacy and economic awareness;
  • and by educating and helping to alleviate problems facing youth in today's society.
  • Assessment and Referral Services

    ARS

    Our Assessment & Referral Services (ARS) provide high-quality, objective health assessments and referrals for individuals with possible clinical problems. Assessment reports are lengthy and include detailed information regarding any substance abuse or mental health issues as well as comprehensive diagnoses on all five axes. We take great care to ensure that our referrals offer the appropriate treatment that can address the individual's unique treatment issues and life circumstances. We work closely with the legal system as well as a variety of treatment agencies within the state. BRC does not provide treatment services. Our Services is dedicated to providing confidential.

    Peer Education Services

    PES

    Bradley Corporation valued education. We provide essential outreach services for students, support groups, classes, workshops, seminar and organizations on a variety of health-related topics.

    Research suggests that people are more likely to hear and personalize messages, and thus to change their attitudes and behaviors, if they believe the messenger is similar to them and faces the same concerns and pressures. Numerous studies have demonstrated that their peers influence youth's health behaviors—not only in regard to sexuality but also in regard to violence and substance use. Peer education draws on the credibility that young people have with their peers, leverages the power of role modeling, and provides flexibility in meeting the diverse needs of today's youth. Peer education can support young people in developing positive group norms and in making healthy decisions about sex.

    Community Integration

    Bradley Corporation helps individuals to optimize their personal, social, and vocational competency to live successfully in the community. Persons served are active partners in determining the activities they desire to participate in. Therefore, the settings can be informal to reduce barriers between staff members and persons served. The under-served individuals we help have the same dreams that we all do. They want to live in their own homes, spend time with family and friends, find meaningful work, and enjoy the many small pleasures of being part of a community.
